It still has the original engine, but the engine bay was taken apart and everything was cleaned and replaced if needed. You can here how clean and strong the engine sounds on the 2 videos I've uploaded. The whole truck is very clean with no rust, dents, or scratches. The tires were recently replaced as well as the muffler and exhaust system.
